Our approach is to use 360 reporting in various forms including extensive team interviews to create the foundation that identifies the need for change.  After identifying the areas that an individual is willing to engage, the sessions are one on one, though there is often a need to observe them in team settings and to supply rapid and direct feedback about performance.  We agree on a plan and we execute against the plan behaviorally and measure the results just as we are expected to do with every other aspect of our work.
